Thank-you for your interest in TC-MRC

TC-MRC has been created to provide an orderly process for optimal deployment of credentialed medical personnel in the event of an emergency.  MRC allows for the maximum use of each volunteer’s skills and expertise.  Even if you have primary medical responsibilities elsewhere, we encourage your participation through the MRC for those times when you will be available to help.  There are certain Federal and Colorado Liability protections for volunteer members of TC-MRC.

TC-MRC Application Process - Please Complete ALL 5 Steps

  1. TELLER COUNTY MRC APPLICATION FORM
    You may complete the application on-line and then print it.

  2. COLORADO VOLUNTEER MOBILIZER - CVM
    Sign-up on the CVM site and print the first page of your completed profile.

  3. TELLER COUNTY EMERGENCY NOTIFICATION LIST - ENL
    You may complete the ENL on-line and then print it.  (We will deliver a copy of the form to the TC Office of Emergency Management.)

  4. Copy of your Colorado Driver’s License

  5. Copy of your Medical License or Certification OR
    Adjunct Medical personnel:  Please provide a statement of your current (within the past year) medical adjunct experience and the name and contact number of your supervisor in lieu of a license/ certification.

Mail or deliver the above 5 documents to the address on the application or convert the documents to pdf form and email them to sirochmans@co.teller.co.us.

YES, WE KNOW this is a multi-step process. 

However, to protect the patients we serve and our fellow volunteers, and to ensure the credibility of the Teller County Medical Reserve Corps, it is imperative that we be thorough and diligent in our screening process.
